As a new patient, and for returning patients, please fill out the forms sent through text when confirming your appointment. Entering information before your appointment helps provide a comprehensive overview of your health and allows us to verify your insurance benefits for a seamless experience. If you are using dental insurance, please bring your member ID card or email us a copy at [email protected].
To examine your teeth efficiently, please make sure to brush and floss before your appointment.
Please arrive at the office at least 15 minutes prior to your appointment. After arriving at Dental Matters, our receptionists will greet you with a warm smile and help prepare for your appointment. When Dr. Syed is ready to see you, an assistant will take you to the operatory to receive a consultation and treatment.
After an examination by our doctor and diagnostic imaging, a treatment plan will be made with approximate costs/insurance benefits and presented to you in order to get started with your treatments. Please ask us any questions you might have at this point.
Once you have a clear understanding of your treatment plan, please schedule your next appointment with our receptionist at the front desk.
