This website (the “Site”) is owned and operated by Humzavis Dental Inc, doing business as Dental Matters (“Dental Matters,” “we,” “us,” or “our”). By using the Site, you agree to be bound by these Terms of Use and to use the Site in accordance with these Terms of Use, our Privacy Policy, and any additional terms and conditions that apply to specific sections of the Site or to services available through the Site or from Dental Matters.

Accessing the Site, in any manner, whether automated or otherwise, constitutes use of the Site and your agreement to be bound by these Terms of Use.

We reserve the right to change these Terms of Use or to impose new conditions on the use of the Site from time to time, in which case we will post the revised Terms of Use on this page and change the Effective Date above. By continuing to use the Site after we post any such changes, you accept the Terms of Use, as modified.

Text Messaging (SMS) Program Terms

Program description. Dental Matters offers a text messaging program to patients and prospective patients who choose to opt in through the contact form on our website at https://dental-matters.com/. We offer two separate types of text messages, and you may opt in to one or both independently of each other:

• Non-marketing messages: appointment reminders, consultation confirmations, and treatment updates.
• Marketing messages: promotional information about our dental services, including dental implant and full arch services.

How you opt in. You opt in by checking the applicable, separate, optional consent checkbox on our website contact form. Each checkbox is unchecked by default. The consent language reads as follows:

Non-marketing messages: “By checking this box, I agree to receive non-marketing text messages from Humzavis Dental Inc DBA Dental Matters about appointment reminders, consultation confirmations, and treatment updates. Message and data rates may apply. Reply STOP to opt out, HELP for help.”

Marketing messages: “I consent to receive marketing text messages from Humzavis Dental Inc DBA Dental Matters at the phone number provided. Frequency may vary. Message and data rates may apply. Text HELP for assistance, reply STOP to opt out.”

Consent is not a condition of service. Your consent to receive text messages is optional. You do not have to agree to receive text messages to schedule an appointment, receive treatment, or purchase any service from us.

Message frequency. Message frequency varies depending on your appointments and the types of messages you have opted in to receive. Text messages are sent from 224-506-1847.

Message and data rates. Message and data rates may apply to messages sent to you from us and to messages sent to us from you. For questions about your text or data plan, contact your wireless provider.

How to opt out. You can opt out at any time by replying STOP to any message you receive from us. After you send STOP, we will send you one confirmation message and you will receive no further text messages from us. To resubscribe, reply START or sign up again through our website form.

How to get help. Reply HELP to any message for assistance, or contact us at 847-227-8277 or 224-506-1847 during business hours.

Supported carriers. Our text messaging program works with all major U.S. wireless carriers. Carriers are not liable for delayed or undelivered messages.

Age restriction. You must be 18 years or older to participate in our text messaging program.

Your information. We do not share mobile opt-in data or phone numbers with third parties or affiliates for their marketing or promotional purposes. Please see our Privacy Policy at https://dental-matters.com/privacy-policy/ for details about how we collect, use, and protect your information.

We comply with all applicable laws and regulations, including the Telephone Consumer Protection Act (TCPA) and CTIA guidelines, regarding the use of text messaging.

Informational Use of the Site

The content on this Site is provided for general informational purposes and is not dental or medical advice. Use of the Site, including submitting a form or sending a text message, does not create a dentist-patient relationship. Please do not include medical or health information in the contact form. If you have a dental emergency, call our office at 847-227-8277 or call 911.

Intellectual Property Rights

Our limited license to you. This Site and all the materials available on the Site are the property of Dental Matters and/or our affiliates or licensors and are protected by copyright, trademark, and other intellectual property laws. The Site is provided solely for your personal, non-commercial use.

You may not use the Site or the materials available on the Site in a manner that constitutes an infringement of our rights or that has not been authorized by us.

Unless explicitly authorized, you may not modify, copy, reproduce, republish, upload, post, transmit, translate, sell, create derivative works, exploit, or distribute in any manner or medium any material from the Site. However, you may download and/or print one copy of individual pages for your personal, non-commercial use, provided that you keep intact all copyright and other proprietary notices.

Your license to us. By posting or submitting any material (including comments, social media posts, photos, and videos) to us via the Site or other digital venues, you represent that you own the material or have obtained the necessary permissions. You grant us a royalty-free, non-exclusive, worldwide license to use, modify, transmit, distribute, and publicly display such material. This license does not apply to your mobile phone number or text messaging consent, which we use only as described in our Privacy Policy.

These Terms of Use shall be governed by and construed in accordance with the laws of the State of Illinois.
